Spaces

We find Him in the spaces. We share Him in the spaces.

David and I have relatively secular jobs. I’m a nurse. He’s a teacher. And while I find nursing, the act of healing and ministering to tired and sick bodies (and by connection to tired and sick souls) to be one of the most spiritual things I ever do, no one labeled me a “missionary” when I worked 12-hour shifts as a nurse for sick foster care kids in Chicago.
 

Our ministry here in Fimpulu is of the subtle daily grind variety, not the door-to-door evangelism or Jesus Film Conference variety. Choshen as a whole does do a significant amount of more obvious evangelism-- they hold training conferences for pastors and teach Bible stories at preschool and a myriad of other awesome things. We love and wholeheartedly support that work and have truly treasured the times we’ve been able to contribute to it in tangential ways, like when David helped build the conference center for the aforementioned pastoral trainings. Nonetheless, we often feel pressure (real or imagined) from those back home to emphasize the “Jesus” in what we do, to share stories of overt evangelism or conversions or whatever. 

And if I’m honest, sometimes that gets to me. Sometimes it makes me feel like I’m not missionary-ish enough.

So I could tell you stories about how Mwewa and Robert come to my door and ask if they can sit on my couch and read the Bible (duh, of course you can, whenever you want to), and I could let you think that these two teenage boys are totally on fire for Jesus and that’s why they’re foregoing the opportunity to play with my soccer ball in favor of the opportunity to read scripture.

And I hope that’s part of it. I do. I hope there’s a little curiosity and wonder brewing in their mischievous little souls, and that they’re absorbing some of the words that they read aloud to me, their cadence painfully choppy and halting the way that it is when one is still trying to grapple with the art of reading. 

But honestly, I think a lot of it is just that they love reading something in Bemba. They love the praise they get from me when they finish a chapter.  They love that I pull that sacred book down from the top shelf where it normally rests, away from the more sturdy children’s books that everyone is allowed play with. I think they feel a little important because I won’t let preschoolers play with the book that they get to read out of. They’re special.

Can that just be okay? Can it just be okay for exposure to the Gospel to wind its way into the little spaces between the normal of their lives? Can it just seep in between discussions of whose turn it is to hold the book and arguments over what an unfamiliar word means and humor over their desire to skip the genealogies because snore? And can it just be okay that those spaces minister to me too, that my own soul is never more at rest than when it shares an ordinary and mundane and undefined space with some village kid and that Still Small Voice?

The simultaneous allegiance to and fear of animism and witchcraft exists simultaneously alongside profession of belief in Christianity for most locals here. Mother’s tie (or allow other’s to tie) charms and “medicines” around their children’s necks or waists. These witchcraft charms are meant to do everything from preventing convulsions to warding off curses. And so when I’m checking over tiny bodies, listening to fevered hearts or percussing consolidated lungs or trying to get a read from my portable pulse oximeter as it dangles from a wiggly and uncooperative finger… When I’m scrubbing burned escar off of mangled limbs or sitting on an uncooperative 2-year-old while I force him to take his medicine or cutting out stitches or cleaning off ill-kept umbilical cords… These little interactions, they create spaces. They create spaces where the breath of Aslan sweeps through. They create spaces that are somehow exactly the shape of my soul, like I was made to fit into it. They create spaces of trust, spaces where momma’s tell me why they don’t feel safe taking off that charm because they fear this or that or the other, spaces for me to talk through how much bigger our God is than all of those other things. Spaces to voice fear, and spaces to drive it out with the Perfect Love that is our Jesus.

I don’t mean to insinuate that all of the charm-loving parents of Fimpulu have started renouncing their traditional ways because of my pleading (they haven’t). If it were that easy, Choshen would have tied the bow on that box a long time ago. But every once in a while, I hear a mom “educate” one of her peers as they wait in the long clinic queue using words that I’ve previously spoken to her. “Don’t dress him. His body is too hot, so wash him with this cool water and fan him. The clothes are like a blanket that make him hot.” Or a co-worker tells a mother that she mustn’t fear the spirits but must put her trust fully in God, because if she knows the power of the blood of Jesus and trusts fully in Him, then no other power has dominion. Or a young momma brings her baby boy in just to say hi, because we fought for him when he was so so so tiny and new and every auntie and grandma in the village was whispering in her ear, but she listened to us instead. And he lived. And we celebrate every ounce of gained weight with a dance and a laugh and Joy— you know, the kind of Joy that fills those kinds of Spaces.

But can it just be okay if the first mom covers her baby up anyway? If the second mom leaves the charm on for now? I mean, obviously it’s not “okay” in the sense that it’s “okay” for all of eternity, but for just the right now, can it just be okay that that they’re still working through the Spaces? 

Those spaces where He finds me.

Those spaces where I find Him.

Those spaces where I yearn to take Him to them.


Those spaces where He whispers to me that He is already there.

Perspective


At Fimpulu Rural Health Center, there is a separate log book or register or tally sheet for pretty much everything. Visits to the outpatient department. Prenatal appointments. Family planning patients. Revisits or follow-ups of any and every kind. HIV drug dispension. TB patient visits. Kids identified as being nutritional deficient. Meds dispensed to the out-patient department, or maternity wing, or counseling/testing room. Malaria tests given.

Ya’ll. There’s probably 50+ registers. No joke. 

This week, a very official-looking woman came and basically audited our registers. After several hours of pouring over cramped and messy handwriting, she came into the screening room where I was working and, with a big smile on her face, proclaimed that “we are not doing well.” 

Turns out, there is yet another register (or three) that we should be keeping, and a line of protocol that goes along with it, that we’re lagging a bit behind in. Specifically, all children should be HIV tested after they are weened, and that test should be recorded in a register so that there is some record that it happened. Because the reality is that right now, it’s not actually happening. And that’s really not okay, in a place where the HIV rate is officially 13% but probably actually painfully higher.

Audit Lady was actually extremely kind and obviously cares about her job and the kids she hopes to help through it. We chatted for several minutes about ways to implement the proposed strategies and protocols. She took my phone number and gave me hers, saying that she had some written resources at her office that she thought would help me out and inviting me to stop by next time I’m in Mansa and pick them up. 

To speak my truth, I deeply passionately dislike the part of nursing that is charting. I don’t enjoy spending literally half of my time on the job just writing in books. I don’t know many nurses who do. It is the bane of nursing existence.

And 6 months ago, if you had told me that there are several dozen books that should be charted in on a daily basis at every rural health clinic in Zambia, I would have scoffed at the inefficient waste of time and the definite double-charting (writing the same information twice, or sometimes 3 or 4 times, in different books) that is surely taking place in such a system.

But then I jumped face first into hot water (that is to say, I started actually working at one such clinic), and my know-it-all-ness has toned down a squeak or two. There’s that old adage that you shouldn’t judge someone until you’ve walked a mile in their shoes. I think it is all too easy to walk a mile in similar shoes (i.e., a nurse who has relatively significant related experience but hasn’t actually worked as a nurse in this particular kind of clinic) and go ahead and cast judgment.

Because yesterday, an HIV+ mom came in to pick up ARV drugs for her HIV+ kids, and we didn’t know what meds to dispense, because their names weren’t on the register. She went home empty-handed with instructions to bring back their paperwork so we can try again.

At the end of every week, I pull about a dozen scraps of paper out of my nursing bag on which I have hastily scrawled the names and identifying information of every clearly malnourished child that I saw that week. I’ve been here long enough that I’m starting to get repeats, and that’s frustrating, because no follow-up is happening. I just keep writing down their names. Last week I found a register that is supposed to be used for that purpose, so that their names can’t be forgotten and dismissed as easily as little scraps of paper that end up left in my pocket and scrubbed to death in the laundry.

So that register of meds? That register is crucial. It means that we know exactly what medication a patient needs, and what medications they may have been on in the past, and what day they picked up their meds last time (and therefore whether they picked this batch up in time to keep from running out). It means that we can calculate how many of each kind of ARV in what doses we are going through in a month to avoid running out. 

And that register of under-5s malnutrition that I found forgotten in a stack of nonsense by the refrigerator? That means that when I encounter a malnourished kid, I can go back and see if they’ve been identified as such before. I can see if follow-up has happened with them. I can tell if they managed to slip under the radar somehow, or if they were fully on the radar and just not addressed.

And the birth register means that if a mom doesn’t show up with her newborn for postnatal within a week of going home, we know who and where and can try to figure out why. 

And when folks come through for the umpteenth time but we can’t find any history of them in any of our registers, it’s a sign to us that the flow of things is failing, and that we as practitioners are letting people slip through the cracks.

So it’s all in perspective. 

Of course, the system isn’t perfect. In the above examples, oftentimes the problem is actually caused by someone's failure to fully or appropriately utilize the registers. Writing it down doesn’t guarantee that anyone will do anything about it. With the frequency of staff turnover and building renovation that has reportedly happened in the last few years, some registers have been neglected or forgotten completely. There are stacks of old registers from years past that I honestly have no idea what to do with and have ceased to become very useful simply because of age and the sheer difficulty of locating anything within them. 

Nonetheless, all of these registers exist for a reason. Sometimes a functional reasoning, and sometimes merely a data-reporting reason, but a reason nonetheless. On more than one occasion, I have thought to myself that this and that procedure or log book or whatever could be consolidated to be more efficient, but the pattern so far has pretty consistently been that when I do some asking and digging, there’s a decent reason for why it is the way it is (even if that reason is just that the people who give the money… USAid, for example… require data to be recorded in a particular way).

For me, it poses an interesting challenge because of my unique position at the clinic. I am functioning pretty much like a full-time employee of the Ministry of Health in terms of my responsibilities, but of course, I am not that thing. We’re here through Choshen Farm, not the government. And there are a host of services and activities I would love to offer that are related but tangential to the services offered by the clinic. I both have a lot of freedom and take a lot of liberty in my position as it currently stands, but I am still functionally beholden to some of the daily doldrums. 

Like the registers.

But until digitalization (and reliable electricity, and computer literacy) sweep the sub-Saharan, I pray that God will keep me humble (and knock me down when I need it) in regards to the way it is and what I might initially perceive to be a “better” or “right” way.  There’s probably a list of ways that I personally could improve in this area. Feel free to send me your suggestions.

Maybe I could write them all down in a register.

Hope Rises

I heard the footsteps coming up the lane to our house, racing the rain, and I hoped futilely that they were making for the bush path that leads to our neighbor’s house (where sleep is not actually a thing). The knock and “odi!” pull me from bed and draw me to the door. One of David’s teacher colleagues is there. She lives by the clinic. She tells me that someone has collapsed. A few minutes later my own footsteps are racing the rain. I think on my way that I am so thankful for the series of events that landed us in our dear house with the purple walls right across from the clinic instead of a decent walk down the road. I also murmur a prayer of thanks for the slight rain, that cools the air and dissuades the mosquitos.

 It takes a good amount of Q&A to figure out what exactly happened and who exactly all the people present are as I try to rouse the unconscious woman. The most easily identifiable culprit, cerebral malaria, falls flat with a negative Rapid Diagnostic Test. I start an IV and run dextrose, trying to balance the possibility of hypoglycemia with the probability of exacerbating a hypertension problem with IV fluids. A car pulls up and the driver informs me that he has brought me a maternity case. I let them into the maternity ward then return to greet the ambulance, who came to pick up the unconscious lady, just as a second maternity case arrives.

Nine females in a room together: Two in labor— a young scared witless first time mom, and a veteran of five previous births. One a toddler and therefore wholly uninterested. One toddler’s mother whose relation to the aforementioned laboring women I’m still not entirely clear on. One wise old grandmother who kept putting gloves on (please don’t put your fingers inside of anyone, Bambuya…). One auntie. Two mothers holding and caressing their exhausted daughters, who appear strangely small and young and in need of their mommies in this moment for two girls about to become mothers, whether for the first or sixth time. And me.

The room smells of that curious mixture between bleach and blood. Both mommas progress slowly through labor at an almost identical pace, and I go through an entire box of gloves flitting back and forth between them. The younger of the two is in utter despair at the task before her but still feisty enough to cop a swing at the auntie who throws a tease her way. The other is taking far longer to progress than I expected, and my gut tells me something is wrong (but you can’t write “gut instinct” as reason for referral, so we press on). They labor through the night. I wonder at what point it is appropriate to begin using the greeting for “good morning”— 3am? dawn? Both of them vomit. Veteran Momma eats and drinks to keep her strength up, but Newbie Momma is scared and tired and won’t even take water. Between sips and checks and reassurances I slip into the back room and try to sleep for little spurts. When sleep won’t come, I pull out my yarn and crochet bunny slippers for the babies that are on their way to meet us, and in every stitch goes a prayer and a song. 

When dawn pushes its way through the dusty curtains that I haven’t gotten around to straightening yet (the string that holds them up is crooked and ohhhh the insult to my OCD), I give up on sleep altogether. My saint-of-a-husband drops by and deposits two pieces of french toast for breakfast on the table in the entryway, where they will remain for several hours until everyone gets done being born and stuff. Newbie momma delivers first, and between her relief and her extended family’s clamor I can feel the frustration and exhaustion of Veteran Momma, who has been laboring longer and surely has paid her dues for long deliveries with the first five births. Blessedly, she follows soon after. The cause of her complication and delay becomes glaringly evident, and I handle it to the best of my ability. The immediate danger is passed, but high risk of infection and Veteran Momma's general poor state after delivery prompt me to send my second patient in 12 hours to Mansa General Hospital. I pray an OB is on duty somewhere and will give her the attention she needs. As the car pulls away, I look down at my clothes and realize that I am wearing the body fluids of no less than 4 people (though 2 of them were less than an hour old, so somehow that doesn’t bother me as much. But still. Meconium everywhere). 

Prego moms and newborn babies are, in my experience, easily the most vulnerable people I encounter here. I’m not exactly sure how many neonatal sepsis cases I’ve dealt with in the last few weeks. Somewhere between 6 and 12. Umbilical cord infection was the obvious culprit in probably 90% of them. A couple weeks ago I carefully wrapped a 2-week-old boy and tearfully handed him to his devastated 18-year-old mother, who alternated between screaming the name of the child she had just lost and crying for her own mother. Not even an hour later, I welcomed a precious 3kg little boy into the world. That same little boy would be back in my arms 6 days later, fighting infection, spending the night with his momma in the same bed in the same ward that he stayed in the day he was born.

There is a deep and terrible beauty to existence here. It is raw. It is communal, and it is fragile. It is the jokes and jests of the half a dozen women who coached Newbie Momma through a slow and painful right-of-passage into adulthood. It is the surprising efficiency with which those same women sprang into action the second the baby was born, cleaning up momma and bed and wrapping up baby and disposing of the placenta. It is the deep searing tragedy of children lost before they ever really even had a chance to be, infants who slip away before they can even be named, as if somehow that namelessness gives them the peace to go, because they aren’t anchored here. 

It is all of those thing. 

And today, blessedly, it is life. It is a dainty precious baby girl and a hearty cone-headed baby boy. It is complications that didn’t complicate as much as they should have. It is improvisation that actually worked and laughter and joy to be shared. Today, it is hope, and it rises both defiant of and oblivious to the statistics or the hard of days past, because that’s just what hope does.

An Introduction to Computers in Fimpulu

Sometimes, in the line of duty, we servants of God and servants of others are required to stretch our knowledge and capacity in ways that we neither expect nor intend. And while for some this means going without food and water, or for others it means leaving behind family without ever seeing them again, for me this means one peculiar thing: teaching computer classes. And though nobody has ever accused me of being tech savvy, I have learned the material, begun to teach it alongside my other classes, and have seen how it represents some of the great problems and opportunities offered in this region.

As most of you know, I came here primarily as a teacher to focus on English, Maths, Civics, and whatever else I could as I became more familiar with the language and the community. When we arrived, the Grade 9 students were preparing for their final standardized examinations that would determine whether they are allowed to go to Grade 10. When we met with the headmaster and with students, they asked only for one subject again and again: computer proficiency classes. You see, in an effort to launch an entire generation of students into the modern world, the Zambian government just added a computer portion to the Grade 9 standardized exams. Students across the country now had to learn (and be taught) facts about computers and some basic tasks like word processing.

Though this goes without saying, in America we have the luxury of taking computer access for granted. From the youngest age, I remember gradually learning how to use a computer from the Macintosh box in 1st Grade, to the Windows 95 hulk in my mom’s classroom, to the XP Desktop in my parents’ house, to the MacBook Pro that I am typing on now. For many in my generation, computers evolved alongside us and, in turn, became an extension of our bodies. Even for our parents, the transition has been adaptable even if not quite as seamless. And even if a computer does not sit in someone’s home, they have access to those at America’s great public library network.

It is understandable, then, that the Zambian government wants to try to catch its students up to the Global North’s level of proficiency. As with much of the world, the Zambian government’s initiatives strive resolutely to improve science and technology education, whether as a way to keep up with global education standards or simply to maintain their vast mining sector. As technology changes at what seems like an ever-increasing rate, it is both responsible and necessary for the national government to take this great leap forward now in requiring students to learn how to use computers.

Nevertheless, as always, the storm of progress hits the poor with the most fury. This is especially true when policy is made by those with wealth, power, and access to vast and diverse resources. Most students in Lusaka and the Copperbelt will not be unduly phased by this new requirement. Wireless internet streams across the larger cities, and Lusaka even opened an Apple store recently. Many Grade 9 students in those regions have computers in their homes or have used them for years.

On the other hand, in the rural areas, very few students have ever even seen a computer, much less used one personally. They are generally too expensive; they are difficult to maintain; they require electricity; they are a luxury. This list is repeated at the rural schools themselves, which generally do not have electricity and certainly do not have their own computers. In fact, only three weeks before the Grade 9 exams, our local primary school received a dozen laptops so that the ninety students could take their tests (thankfully, I was able to use one to teach the students during that short time). The students would be tested on basic definitions as well as using Windows 7 and printing a typed page from Microsoft Word. The computers we received in Fimpulu did not come with a printer, were mostly XP, and did not all already have Microsoft Office loaded. But in their frantic and honorable work, the teachers hooked up their diesel generator, loaded the right software, and acquired a printer from town. How other schools individually solved their systemic problems, I do not know. How other underprepared students completed the demands placed before them by their distant directors, I do not know. Such is life in the periphery.

But with all of these difficulties faced in the rural areas—all of the inefficiencies, all of the setbacks, all of the roadblocks, and all the demands of those in power, there is no question that computer access is important not only to fulfill a requirement but also because—even more now than it was 10 or 20 years ago—the computer offers a real tool for revolution in rural Africa, and specifically for the type of revolution required today in the neocolonies. The laptop especially is a profoundly democratizing and empowering tool, both for education and practice after the fact. Though its benefits are still not entirely in reach, logistical roadblocks to laptop access are slowly wearing down. Cellular phones are in almost every person’s hand here, as is a 2 or 4 GB MicroSD in that phone that provides a foundation for using and sharing information on a large scale. Electricity has gradually become less difficult to access, as solar panels become cheaper, easier to purchase, higher quality, and much more efficient. Despite the fact that the national electric grid has not reached Fimpulu, a laptop computer can get its 6 hours of charge off of an hour or two of sunshine. Along with cellular towers has come passable internet connectivity, so that you can buy an internet dongle and periodically reload it with data. Finally, as we all know, laptops themselves are becoming a better value. Although a laptop computer is still inaccessibly expensive for the average working person in a village, it is possible to purchase one only a short car or bicycle ride away. A netbook in Mansa costs about $300, which equals somewhere between two and six months wages for the average rural worker before they subtract the cost of raising a family and sustaining oneself. Largely, this cost problem is addressed through internet cafes that offer paid computer, downloading, and copy/print services. However, as access and knowledge about computers improves, it also opens the possibility of socialized ownership of one computer that can be divided like any public utility. One laptop or netbook, spread among twenty or fifty users, can still provide the amount of the resource needed by people in the village. Structured like a library, such access could, theoretically, help to spread the benefits of computers wide enough to make it both affordable and convenient.

With these hurdles on the way out, what then are the benefits of computers in the village? They are ones we all know, yet they have become much clearer to me here. For experienced users, the internet offers the whole world at their fingertips, whether for research, communication, or entertainment. While some material costs money, and all of it costs data, the extensive free material online offers an opportunity akin to the invention of the printing press in Europe. This is equally true for documents and essays already downloaded. I have an extensive collection of African, Africanist, philosophical, and religious books and articles on my computer and hard drive that can be copied for free forever. Though even in America I believe that intellectual property is public property, here it does not have to be defended so stridently; piracy is such a harsh word for social uplift. Thus, while books here are comparatively expensive and almost always must be imported, downloads are free and endlessly reproducible. When I held up a Micro SD, pointed to the bookshelves in the back of the room, and told my students that they could fit all those books on their phones, they were appropriately amazed, as we all should be. Especially for classroom materials and curriculums, digitization can (and will) profoundly improve the state of education in rural Zambia and other outlying places. Finally, digitization, like automation, offers the chance to significantly improve business and accounting abilities even on the village level. Whether for small businesspeople and shop owners or for farmers and co-ops, access to programs like Word and Excel or to internet research about farming practices clearly improves quality of life; shared communally, these benefits only increase.

With this in mind, I have eagerly accepted that strange responsibility that I did not expect or intend. For my own part, I tutored the Grade 9 students to the best of my ability with the resources we had available in our academic triage environment. I plan to teach them over the next year in more formal computer classes at the school, hopefully using both English and Bemba to better explain the concepts and controls. Currently, I am offering informal classes for both school-age and adult learners at Choshen Farm’s Learning Resource Center. These have been a pleasure to hold, especially for the adults who would have no other convenient and free opportunity to learn about and use a computer. The lesson on “how much data you can store in what” is equally amazing for the 15-year-old and the 50-year-old. I also already have a queue of requests for book downloads about Africa or by Kenneth Kaunda, their first president.

In the midst of all the negativity and fear coming out of Africa, or all of the false optimism that covers over the real struggles of the poor, there are some genuine moments of inspiration and opportunity. I will not pretend that there is not a hard long road ahead—Grade 9 students in poor communities will continue to struggle for lack of resources and time as they become the unsuspecting vanguard for development; technological progress, national policy, and the neo-liberal fight to privatize the commons will continue to erect new challenges. But as this technology and its benefits spread through the continent, those on the continent are increasingly better enabled to fight back and push forward.
